TMJ, TMD, Clenching & Tooth Grinding

The temporomandibular joints, commonly called the TMJs, connect your lower jaw to your skull. You have one joint on each side, located just in front of the ears.

Problems involving the jaw joints, chewing muscles, and surrounding tissues are more accurately called temporomandibular disorders, or TMDs.

Tooth grinding and clenching are known as bruxism. Bruxism can happen while you are awake or asleep and may contribute to tooth wear, damaged restorations, sensitivity, or muscle soreness.

Jaw symptoms have many possible causes. An evaluation helps us understand what you are experiencing before recommending a next step.

Signs Worth Discussing With a Dentist

  • Jaw pain or tenderness
  • Jaw stiffness or limited movement
  • Painful clicking, popping, or grinding sounds
  • Morning jaw muscle soreness
  • Worn, flattened, chipped, or sensitive teeth
  • Repeated damage to fillings or other dental work

What to Expect During a Jaw or Tooth Grinding Evaluation

We begin by understanding your symptoms and checking for signs that may help explain what is happening.

Tell Us What You Notice

We ask about jaw discomfort, grinding, clenching, headaches, tooth sensitivity, sleep-related concerns, and when your symptoms occur.

Examine Your Teeth and Jaw

The dentist checks the teeth for wear or damage and evaluates jaw movement, tenderness, and other relevant findings.

Review the Findings

We explain what we observe and whether the concern appears related to grinding, dental damage, jaw function, or another issue that needs additional evaluation.

Discuss the Next Step

Depending on your findings, we may discuss conservative care, protection for your teeth, an appropriate appliance, restorative treatment, monitoring, or referral.

There is no single test or treatment for every jaw problem. Persistent or complex symptoms may require evaluation by another dental or medical professional.

Conservative Care and Protecting Your Teeth

Treatment depends on what is causing the symptoms and which parts of the teeth, muscles, or jaw are involved.

Protect Worn Teeth

If grinding or clenching is damaging your teeth or restorations, we can evaluate the amount of wear and discuss ways to help protect them.

Conservative Management

Depending on your symptoms, simple measures such as reducing clenching habits, avoiding unnecessary jaw strain, and other conservative approaches may be discussed.

Oral Appliance When Appropriate

An oral appliance may be considered for selected patients, particularly when protecting teeth from grinding or clenching is part of the treatment goal.

An appliance is not a guaranteed cure for jaw pain. Recommendations are based on your individual symptoms, teeth, and examination findings.

Not Every Jaw Click or Headache Is a TMJ Disorder

Jaw clicking without pain or difficulty moving the jaw is common and may not require treatment.

Headaches, ear discomfort, facial pain, and jaw soreness can also have causes unrelated to the jaw joint. Our role is to evaluate dental findings and help determine an appropriate next step rather than assuming every symptom has the same cause.

Jaw symptoms can have multiple causes Painless clicking may not need treatment Grinding may occur during sleep or while awake Tooth wear can sometimes reveal grinding Conservative care is often considered first Referral may be appropriate for complex symptoms

When Should You Have Your Jaw or Teeth Checked?

Consider scheduling a dental evaluation if jaw discomfort, clenching, grinding, or tooth wear continues or begins interfering with eating, sleeping, or daily activities.

You should also call if you notice tooth damage that may be related to grinding or clenching.

Questions Patients Often Ask

What is the difference between TMJ and TMD?

TMJ refers to the temporomandibular jaw joint. TMD refers to a group of conditions that can affect the jaw joints, chewing muscles, and surrounding tissues.

What are common signs of a TMJ or jaw problem?

Possible symptoms include jaw pain, tenderness, stiffness, limited movement, locking, or painful clicking and popping.

What is bruxism?

Bruxism is clenching or grinding of the teeth. It can happen during sleep or while awake, and some people are unaware that they do it.

How can I tell if I grind my teeth?

Signs may include worn teeth, chips, damaged restorations, sensitivity, jaw soreness, or someone hearing grinding while you sleep.

Does jaw clicking always need treatment?

No. Clicking without pain or limited jaw movement is common. Painful clicking, locking, or persistent difficulty moving the jaw should be evaluated.

Can a night guard help with tooth grinding?

An oral appliance may be appropriate for selected patients to help protect teeth from grinding or clenching. The recommendation depends on your examination.

Can grinding damage my teeth?

Repeated clenching or grinding can contribute to tooth wear, chips, fractures, sensitivity, or damage to existing dental restorations.

Can TMJ problems cause headaches?

Some TMD conditions occur with headaches or facial discomfort, but headaches have many possible causes and should not automatically be attributed to the jaw.

Does insurance cover TMJ care or night guards?

Coverage varies significantly by dental and medical plan and by the treatment recommended. Contact our team with plan-specific questions.
